Home Care Assessment
Our registered nurse visits the senior's home to conduct a comprehensive assessment of health status and care needs — covering physical function, daily living abilities, chronic disease management and the home environment — then drafts a personalised care plan so families clearly understand their loved one's real needs. Elderly holding a Government Community Care Voucher enjoy a free on-site assessment; for non-voucher elderly, an on-site nurse assessment is charged at a standard fee.

Registered Nurse Home Care
Professional on-site nursing by registered nurses and trained carers — covering personalised care for elderly with cognitive impairment, medical appointment escort, short-term respite care for family caregivers, personal hygiene care and regular medication management, plus 24-hour daily living support and overnight watch — providing all-round support for ageing in place.

On-site Physiotherapy
Registered physiotherapists provide personalised on-site physiotherapy for seniors recovering from stroke, fracture surgery or long-term bed rest — including joint mobilisation, muscle strengthening, balance and gait training and pain management — helping them gradually restore or maintain physical function and quality of life.

On-site Occupational Therapy

On-site Speech Therapy
Speech therapists provide on-site assessment and training for seniors with swallowing or communication difficulties — including swallowing safety evaluation, oral-motor and voice training, cognitive-communication training and feeding advice — to prevent choking, pneumonia and malnutrition, so seniors can eat safely and communicate clearly.

Post-op, Pressure Ulcer & Tube Care
Registered nurses provide all-round support for post-discharge and post-operative care — including wound and pressure ulcer (bedsore) treatment and prevention, regular medication management, and professional procedures such as changing nasogastric (feeding) tubes and urinary catheters — helping discharged seniors transition home safely and reducing complications and readmission risk.

CCSV Home Services
